SURVEY

The survey linked above is to be completed by each student at the end of both their Practicum and Internship.  It is an anonymous survey of your satisfaction regarding your placement.  For those that are graduating, you will need to print the final page of the survey (the completion confirmation page), and bring a copy of the page to your exit interview.
