STUDENT EVALUATION OF A CLINICAL PLACEMENT
The survey linked above is to be completed by each student at the end of both their Practicum and Internship. It is an anonymous survey of your satisfaction regarding your placement. For those that are graduating, you will need to print the final page of the survey (the completion confirmation page), and bring a copy of the page to your exit interview.
COMPLETER'S SURVEY
This survey is to completed by every person graduating from the program. It is divided between two parts. Part one captures information about you; part two is an anonymous survey. You will be prompted to print the final page of the survey as confirmation of your completion. You will need to bring confirmation of your completion of the survey to your exit interview.
STUDENT RATINGS OF ADVISING
This survey should be completed annually by every student in the program. This survey was first introduced in the Spring of 2017.
